Required before any personal device (phone, tablet, or computer not owned by Neuroscale) is used to access Neuroscale resources, and re-signed on material policy change. Workers who use only Neuroscale-owned devices do not need this form. See the Information Security Policy → Device policy — company-owned and BYOD and the Employee Privacy Policy.

Scope

This consent covers each personal device I use to access Neuroscale email, Slack, SharePoint, Linear, the Arbi platform, or any other Neuroscale resource. I will list each device in §4. I, {{worker_name}}, in my capacity as {{worker_role}}, acknowledge and agree:

1. Enrollment and minimum controls

a. I will enroll each personal device in Neuroscale’s mobile management (Rippling MDM/MAM) before using it for work, and keep it enrolled while I use it for work. b. Each enrolled device will meet the minimum controls in the Information Security Policy → Device policy: full-disk or device encryption enabled; automatic screen lock after no more than 5 minutes of inactivity with a passcode/biometric; current, vendor-supported OS with security updates applied; endpoint protection where required; and no jailbroken, rooted, or otherwise security-compromised devices.

2. Containerized management and remote wipe

a. Neuroscale manages company data and applications on my device through a managed container or application-management profile. I understand Neuroscale’s management is scoped to company data and does not give Neuroscale ownership of my personal content. b. I consent to Neuroscale remotely removing the company container and company data (“selective wipe”) from my device — without further notice — if my device is lost or stolen, if my engagement ends, if my role changes such that I no longer need the access, or if the device falls out of compliance with the controls in §1. I understand a selective wipe targets company data; I am responsible for backing up my personal content. a. I acknowledge that my use of Neuroscale systems and the company container on my device is logged and monitored as described in the Employee Privacy Policy → Monitoring and the Logging & Monitoring standard. Monitoring is limited to company data, company applications, and access to Neuroscale systems; Neuroscale does not monitor my personal photos, messages, or browsing outside the company container. b. I have received the state-specific electronic-monitoring notices applicable to my work location (including, as applicable, California, New York (N.Y. Civ. Rights Law §52-c), Connecticut (Conn. Gen. Stat. §31-48d), and Delaware (19 Del. C. §705)) set out in the Employee Privacy Policy → State-specific notice, and I consent to the monitoring described there as a condition of BYOD access.

5. Loss, theft, and separation

a. I will report a lost or stolen enrolled device immediately to security@neuroscale.ai per the Asset Management Policy → Loss or theft of assets and the Incident Response Plan. b. On separation or on request, I will allow removal of company data from my devices and will not retain Neuroscale Confidential Information on any personal device, per the Offboarding Procedure.
